Risk logs with different actions taken

Hi,

why does SEPM console show different actions for same Risk name for same workstation. 

the first action taken is Quarantined for the same file name and file path, then within few seconds, it shows another log with action taken as "Left Alone" for the same scenario. how can we consider this risk as? Symantec has quarantined the file or have not yet taken any action? the risk has been detected during Scheduled scan.
