Report to show patch installation count

Hello,

I need help to create a report to show the count of patch installed to all computer in my farm. Below is the criteria.

1- Computer Name

2- Description

3- Installed By

4- Installed Date

5- Patch ID

I need all this with the option to adjust date where i can choose which date range i want. Please help as my customer needs the report asap.
